Exercise during GLP-1 treatment: strength, cardio and starting safely

Physical activity guide during GLP-1 treatment to protect function, strength and health.

Summary and conclusions

The essentials in less than a minute

  • Physical activity provides benefits even if the scale does not change that week.
  • Strength work is important to preserve muscle mass and function during weight loss.
  • Walking, biking, swimming, and other aerobic activities can progress gradually.
  • Chest pain, fainting, severe shortness of breath, or new symptoms require evaluation before continuing.

Why moving is still essential

The authorisations of Wegovy and Mounjaro place them as a complement to a diet with fewer calories and greater physical activity. Exercise improves cardiovascular capacity, strength, mobility, mood and functional maintenance; It is not simply a tool to “burn off” what has been eaten.

The WHO reminds that any amount of activity is better than none and that strengthening benefits everyone.

A balanced plan

Components of an active week
ComponentExamplesProgression
AerobicWalk fast, bike, swimIncrease time before intensity
StrengthMachines, bands, weights, body weightLarge muscle groups 2 or more days
MobilityJoint movements and gentle stretchesRegular, without acute pain
daily lifeStairs, walks, active movementsReduce sedentary time

How to start if you haven’t trained for a while

1Base10–20 minutes of movement tolerable.
2RegularityRepeat before adding intensity.
3StrengthSimple technique and gradual progression.

If there is nausea or low intake, avoid demanding sessions at times of worse tolerance. Hydrate yourself, do not train on an empty stomach out of obligation and review your medication if there is a risk of hypoglycemia.

Stop signs

Stop exercise and seek evaluation for chest pain, fainting, disproportionate respiratory distress, persistent palpitations, neurological weakness, severe abdominal pain, or signs of dehydration.

Mild muscle pain at the beginning is not the same as acute joint pain or progressive loss of strength. The plan can be adapted; It doesn’t have to be extreme to be effective.
