Premature ejaculation: assessment and treatment options
Medical guide on ejaculatory control, causes, assessment, treatments and realistic expectations.
The essentials in less than a minute
- The diagnosis does not depend only on counting minutes: control, discomfort and the impact on the relationship matter.
- It can be lifelong or appear later; In the acquired form it is advisable to look for associated causes.
- Treatment can combine sexual education, psychosexual techniques and medication when indicated.
- It is not a personal failure and asking for help soon can reduce anxiety and tension as a couple.
What it really means
The guides assess time to ejaculation, feeling of control, discomfort, and interpersonal difficulty. An isolated episode or an unrealistic expectation is not enough to establish a diagnosis.
Lifelong or acquired
| Pattern | What is reviewed |
|---|---|
| From the first relationships | Control, frequency, expectations and response to specific options. |
| Appears after a normal stage | Erectile dysfunction, prostatitis, anxiety, thyroid, medication and relationship. |
Medical and sexual history is usually the basis. Routine testing is not necessary in all cases; They are requested when the history or examination points to a specific cause.
Available treatments
European guidelines recommend treating erectile dysfunction, a genitourinary infection, or another identified cause first. The combination of psychosexual intervention and pharmacological treatment may be useful in certain cases.
Safety and expectations
Improvised anesthetic sprays and antidepressants should not be used without assessment. The medication has contraindications, interactions and adverse effects. It is also important to agree on what a realistic improvement would be: more control, less distress and a more satisfying sex life, not a “perfect” duration.
Check if there are other symptoms
Pain, blood in semen, discharge, urinary difficulty, loss of erection, or a sudden change warrant further evaluation.
