Finasteride vs dutasteride for hair loss: what really differs
Differences between finasteride and dutasteride in male alopecia, comparative evidence, authorisation in Spain and safety.
The essentials in less than a minute
- Both inhibit 5-alpha-reductase, but do not block exactly the same isoenzymes.
- In Spain, finasteride 1 mg has an indication for male androgenetic alopecia; Dutasteride 0.5 mg is approved for benign prostatic hyperplasia, not hair loss.
- Short trials have observed greater growth with dutasteride in some groups, but this does not replace an individual decision nor does it make its use authorised for alopecia.
- Sexual, reproductive, and mood warnings require clinical conversation.
Same system, different intensity
Finasteride primarily inhibits 5-alpha-reductase type II. Dutasteride inhibits types I and II and reduces dihydrotestosterone more intensely. This pharmacological difference explains the interest in dutasteride, but greater hormonal suppression does not in itself equate to a better option for each patient.

Safety considerations that should not be minimised
The EMA confirmed suicidal ideation as an unknown side effect of finasteride tablets. For dutasteride it did not establish a direct link, but it will add preventive information on mood changes by sharing mechanism. Known sexual problems should also be reviewed.
Damaged capsules should not be handled during pregnancy and both medications can affect markers such as PSA, something relevant for other professionals caring for the patient.
Do not change on your own
Switching from finasteride to dutasteride is not a simple “step”. It requires reviewing indication, risks, prior treatment and follow-up.
A clinical, non-commercial choice
If finasteride is tolerated and stabilizes the loss, the most potent option is not necessarily the best. If the response is insufficient, diagnosis, time, adherence and photographs are first checked. Dutasteride can be discussed in selected contexts, but should not be presented as an authorised routine hair treatment in Spain.
